High Priority Issues

Voice Recognition Accuracy in Noisy Environments

Issue: Voice transcription accuracy drops below 85% in environments with significant background noise.

Affected Areas:

  • Emergency departments during peak hours
  • Operating rooms with equipment noise
  • Patient rooms with multiple conversations

Workarounds:

  • Use noise-canceling headsets or lapel microphones
  • Move to quieter areas when possible
  • Speak closer to the microphone (6-12 inches)
  • Use push-to-talk functionality to avoid background noise

Status: Fix planned for v2.1 (Q2 2024)

  • Enhanced noise filtering algorithms
  • Improved microphone sensitivity controls
  • Background noise suppression features

Mobile App Battery Drain on iOS

Issue: iOS mobile app consumes excessive battery when voice recording is active for extended periods.

Affected Versions: iOS app v1.8.x and earlier

Symptoms:

  • Battery drain 2-3x normal rate during voice sessions
  • Device heating during extended recording
  • App crashes on older devices (iPhone 12 and earlier)

Workarounds:

  • Close other apps during voice recording sessions
  • Reduce screen brightness during recording
  • Use external power source for extended sessions
  • Update to latest iOS version

Status: Fixed in v1.9.0 (Released March 2024)

  • Optimized audio processing algorithms
  • Reduced background CPU usage
  • Improved memory management

Medium Priority Issues

EMR Integration Sync Delays

Issue: Occasional delays (5-15 minutes) in syncing clinical notes to EMR systems during peak usage hours.

Affected Systems:

  • Epic integrations during shift changes (7 AM, 7 PM)
  • Cerner systems with high concurrent users (>100)
  • Custom EMR integrations with rate limiting

Workarounds:

  • Manual sync option available in note editor
  • Batch sync during off-peak hours
  • Priority sync for urgent/critical notes

Status: Optimization in progress

  • Enhanced queue management system
  • Improved error handling and retry logic
  • Load balancing for peak usage periods

Dashboard Widget Loading Issues

Issue: Some dashboard widgets fail to load or display outdated data intermittently.

Affected Widgets:

  • Real-time patient feedback metrics
  • Task completion statistics
  • Department performance indicators

Symptoms:

  • Blank widget areas
  • "Loading..." state that doesn't resolve
  • Data from previous day showing as current

Workarounds:

  • Refresh browser page (Ctrl+F5 or Cmd+Shift+R)
  • Clear browser cache and cookies
  • Switch to different dashboard layout temporarily

Status: Under investigation

  • Improved caching mechanisms
  • Enhanced error reporting
  • Widget-level refresh capabilities

Multi-language Voice Recognition Switching

Issue: System occasionally fails to automatically detect language changes mid-conversation.

Affected Languages:

  • Spanish-English code-switching
  • Hindi-English mixed conversations
  • French-English bilingual interactions

Workarounds:

  • Manually select language before recording
  • Use separate recording sessions for different languages
  • Speak one language at a time when possible

Status: Enhancement planned for v2.2

Low Priority Issues

PDF Export Formatting

Issue: Generated PDF reports occasionally have formatting inconsistencies.

Symptoms:

  • Misaligned tables in analytics reports
  • Font size variations in clinical notes
  • Page break issues in long documents

Workarounds:

  • Use alternative export formats (Excel, CSV)
  • Print to PDF from browser for better formatting
  • Adjust browser zoom to 100% before exporting

Status: Cosmetic fix planned for v2.0.1

Search Function Case Sensitivity

Issue: Global search requires exact case matching for some medical terms.

Examples:

  • "COPD" finds results, but "copd" does not
  • "McBurney" works, but "mcburney" fails
  • Acronyms require exact capitalization

Workarounds:

  • Use proper capitalization for medical terms
  • Try multiple case variations
  • Use partial word searches

Status: Enhancement in development queue

Browser-Specific Issues

Chrome Browser Issues

Memory Usage with Large Patient Lists

Issue: Chrome browser memory usage increases significantly when viewing patient lists with >1000 entries.

Symptoms:

  • Browser becomes sluggish after 30+ minutes
  • Tab crashes on systems with less than 8GB RAM
  • Scroll performance degrades

Workarounds:

  • Use pagination controls to limit displayed results
  • Close other browser tabs
  • Restart browser periodically
  • Use filters to reduce displayed data

Status: Optimization planned for Q3 2024

Safari Browser Issues

Voice Recording Permissions

Issue: Safari occasionally requires repeated permission grants for microphone access.

Symptoms:

  • Permission dialog appears multiple times per session
  • Recording fails silently without error message
  • Microphone icon shows as blocked despite permissions

Workarounds:

  • Grant permissions at system level (System Preferences > Security)
  • Use Chrome browser for voice features
  • Clear Safari permissions and re-grant

Status: Safari compatibility improvements in progress

Firefox Browser Issues

File Upload Progress

Issue: File upload progress indicators don't update correctly in Firefox.

Affected Features:

  • Profile picture uploads
  • Document attachments
  • Bulk user import files

Workarounds:

  • Wait for completion notification instead of relying on progress bar
  • Use smaller file sizes when possible
  • Switch to Chrome for file upload operations

Status: Firefox-specific fixes planned for v2.0.2

Mobile App Issues

Android App Issues

Notification Delivery Delays

Issue: Push notifications arrive 2-5 minutes late on some Android devices.

Affected Devices:

  • Samsung devices with aggressive battery optimization
  • Huawei devices with custom Android versions
  • Devices with third-party battery management apps

Workarounds:

  • Disable battery optimization for MedFeed app
  • Add MedFeed to "never sleeping apps" list
  • Check notification settings in device settings

Status: Working with device manufacturers on optimization

Offline Mode Sync Issues

Issue: Data created in offline mode occasionally fails to sync when connection is restored.

Symptoms:

  • Tasks created offline don't appear in web version
  • Voice recordings remain local only
  • Patient updates not reflected in EMR

Workarounds:

  • Force manual sync from app settings
  • Ensure stable internet connection before going offline
  • Verify sync completion before closing app

Status: Improved offline sync in v2.1 mobile release

iOS App Issues

Face ID Authentication Failures

Issue: Face ID authentication fails intermittently, requiring password entry.

Conditions:

  • Low light environments
  • Wearing masks or face coverings
  • After iOS system updates

Workarounds:

  • Use Touch ID or passcode as backup
  • Ensure adequate lighting for Face ID
  • Re-enroll Face ID after iOS updates

Status: Enhanced biometric handling in development

Integration Issues

Epic EMR Integration

Order Entry Delays

Issue: Orders placed through MedFeed take 10-30 seconds to appear in Epic.

Affected Order Types:

  • Laboratory orders
  • Radiology requests
  • Medication orders

Workarounds:

  • Verify order placement in Epic before proceeding
  • Use Epic directly for time-sensitive orders
  • Check MedFeed integration status before ordering

Status: Working with Epic on API optimization

Cerner Integration

Patient Matching Accuracy

Issue: Patient matching occasionally fails for patients with similar names or demographics.

Risk Factors:

  • Common names (John Smith, Maria Garcia)
  • Patients with multiple MRNs
  • Recent demographic changes

Workarounds:

  • Verify patient identity using multiple identifiers
  • Use MRN when available for patient lookup
  • Double-check patient details before documentation

Status: Enhanced matching algorithms in testing

Performance Issues

Peak Usage Slowdowns

Issue: System response times increase during shift changes and peak usage periods.

Peak Times:

  • 7:00-8:00 AM (day shift start)
  • 7:00-8:00 PM (night shift start)
  • 12:00-1:00 PM (lunch documentation catch-up)

Workarounds:

  • Schedule non-urgent tasks for off-peak hours
  • Use cached data when available
  • Prioritize critical functions during peak times

Status: Infrastructure scaling in progress

Large File Upload Timeouts

Issue: Uploads of files larger than 50MB may timeout or fail.

Affected File Types:

  • High-resolution medical images
  • Long audio recordings (>30 minutes)
  • Large PDF documents

Workarounds:

  • Compress files before uploading
  • Split large files into smaller segments
  • Use alternative file sharing methods for very large files

  • Critical: System unavailable or data loss risk
  • High: Major feature impacted, workaround available
